Care at Home
The Care at Home Program allows medically fragile children to be cared for at home, keeping the family together and the child involved in family life. The Care at Home program was pioneered at St. Mary’s and serves as a model for other programs in New York State. Comprehensive approaches to case management includes evaluating and assisting with a child’s therapeutic, medical, educational, and psychosocial needs, working with medical equipment and home care personnel and accessing resources to provide appropriate care within the home setting. Home adaptation and vehicle modification are also available to improvement movement and/or safety for the child at home and to improve access from home to the community. Caregivers also take the time to train families in the necessary equipment and care techniques.
A special Medicaid waiver program, Care at Home is designed for families who would not normally be eligible for Medicaid and helps pay for in-home services for medically fragile or severely disabled children whose families would otherwise be unable to pay for these services.
