St. Mary's Remote Patient Monitoring

The use of remote patient monitoring makes it easier for children to transition home sooner and reduces hospital admissions and emergency room visits.

St. Mary’s understands that medically fragile children and young adults have unique needs and want to be active participants in their own care.

Remote Patient Monitoring creates a partnership between the patient, their caregiver, and the St. Mary’s Telehealth Team to allow for real-time communication and responses to changes in condition that help patients avoid unnecessary hospitalizations and emergency room visits. Patients learn to recognize the early signs that indicate a change in their condition that might need attention. Real-time alerts trigger contact by the St. Mary’s Home Care nurse to determine the need for an intervention.

Remote Patient Monitoring (RPM)

Patients receive a remote monitoring kit with peripheral devices to collect their vital signs and to help them manage their health. Bluetooth-enabled devices transmit data to a platform the St. Mary’s Telehealth Team monitors. Alerts are triggered when patient responses fall outside physician-ordered parameters. Appropriate interventions are applied, and patients/caregivers fully participate in their own care. The following biometric measurements can be monitored: blood pressure, heart rate, temperature, weight, and medication adherence. The St. Mary’s Telehealth Team monitors the patient’s progress on a clinical
dashboard and responds to alerts that may indicate a serious change in the patient’s condition.

Virtual Visits

Patients and their caregivers can
receive audio/video calls from their
tablet or cell phone and connect with the St. Mary’s Telehealth Team.

Interactive Voice Response (IVR)

Patients/Caregivers can receive
automated calls that allow them to
answer questions about their health.
Patient/Caregiver responses are
recorded and transmitted to a clinical dashboard. St. Mary’s Telehealth Team
responds to urgent alerts, changes in condition, or requests for assistance.
