at Home
At St. Mary''s Healthcare System for Children experts in clinical case management
work closely with referring providers, families, and agencies to assess each child''s
needs, coordinate appropriate services, and facilitate cost-effective outcomes.
St. Mary''s ensures each patient receives high-quality, goal-oriented medical
and rehabilitative care.
St. Mary's is an approved case management provider for several programs including
Care at Home and Medicaid Service Coordination.
The Care at Home program is designed to meet the needs of
children and families whose home care needs are not fully covered by private insurance,
but who are not eligible for Medicaid. Established in 1986, this program was pioneered
at St. Mary's and has served as a model for other programs in New York State.
Currently, St. Mary's services a large percentage of the New York State
Care at Home population.
The Medicaid Service Coordination program supports Medicaid-active families
with children who require medical, social, or rehabilitative care with expert
case management.
Programs and Services We Offer
Our comprehensive approach to case management includes, but is not limited to:
• Evaluating and assisting in the management of the child's therapeutic,
medical, educational, and psychosocial needs
• Working with medical equipment, supply vendors, and home care personnel
• Accessing resources for care
• Assisting the family in problem solving and decision making
• Advocating with school districts and other government agencies and
third-party payers on the child's and family's behalf
In accordance with a physician's formal treatment plan, experienced case managers
facilitate the family's entitlement to care and social services for their child
with special needs. Such services include:
