Long Term Home Health Care Program (LTHHCP)
St. Mary’s is the largest provider of long-term home healthcare services for children in New York State. Also known as the Lombardi program, St. Mary’s LTHHCP caters to children with a variety of chronic, complex medical conditions who require home healthcare services over an extended period of time. We promote a customized care plan and health management options for children who are in need of support, clinical, rehabilitative, or medical care.
